Kindergarten Roundup Information
Dates: April 23 & 24, 2008
Where: Kindergarten Corral (Board of Education Room), Education Center, 901 S. 4th St., DeKalb
When: Noon until 7:00 p.m.

More than 300 families registered during Roundup. Welcome to our newest young learners! If your kindergarten student (five years of age prior to September 1, 2008) please contact the Education Center for information: (815) 754-23590.

What do I need? Registration packets are available at all District 428 schools and the Education Center. Your registration experience will be enhanced if you have all necessary forms with you and completed when you arrive (delays are possible, please allow plenty of time).

How much will it cost? Currently, payment may be made by check or credit card (checks should be made payable to: DeKalb District 428). Online payment will soon be an option as well.

Online Fee Payment Information for 2008-09 School Year
Returning Students Only in Grades 1-8 

IMPORTANT! 04/24/08 - We have discovered a technical issue involving third party software providing the ability to register online. Until this technical issue is resolved, we ask that you continue to bring/mail your registration information and payment to you school or the Education Center, 901 S. 4th St., DeKalb, IL 60115. We apologize for any inconvenience.

Parent/guardians of returning students should have received registration packets in the mail in mid-April. They are also available now at each District 428 school, or the Education Center, 901 S. 4th St., DeKalb. Preprinted registration forms can be returned to the Education Center. Fee discount is available until May 5th.

If you wish, you may register online, following the simple steps below:

1) Connect to Family Access here (User name and password required. If you do not have, or do not remember user name and password, please contact your school office. Verify that address and contact information is updated and accurate). Until payment feature is activated, please complete student fee form and return/mail with payment to the Education Center. Checks should be made payable to DeKalb District 428. Fee discount is available until May 5th.

2) Complete health form (either use form mailed to your home or download a copy here. This form must be returned to the school for registration to be complete. Your student will not receive a class schedule until this form is received by the school.

Note: You are now also able to add funds to your student's lunch account through Family Access.

Parents/guardians of students NEW to District 428 must register in person at the Education Center, located at 901 S. 4th St, DeKalb. The following items will be needed to register (registration packets may be picked up at the Education Center or at any District 428 school):

1) Registration form (both sides)

2) Proof of Residency (Available at time of registration)

Includes affidavit, and at least one document from either Category A or Category B:

Category A

Real Estate Tax Bill; Mortgage Papers; Signed Lease Agreement; Agreement of Sale; Affadavit from local resident attesting resident is living with the owner at no cost.

Category B (one document, dated within the last six months, showing address of record)

Home/apartment insurance papers; Credit Card Bill, Gas or Electric Bill; Public Aid Card; Water Bill; or Cable Bill

Category C (none of the above documents are applicable because:

• The student is homeless and eligible under the Illinois Education for Homeless Act.
• The student is enrolling based upon the determination of the Department of Children and Family Services (DCFS) (Documentation from DCFS required).
• Residency will be established within 30 calendar days (real estate contract, lease or closing statement required).

An affadavit of custody will need to be indicated on the form as well. It is helpful to have a copy of any court/guardianship documents to attach to the residency form.

4) Program Participation and Release Form (available at registration)

5) Home Language Survey

6) Student Health Form (the Student Health form must be completed and turned into the school or Education Center, 901 S. 4th St., DeKalb for your registration to be complete)
